"Monitoring heart failure patients with telehealth systems has become increasingly important in recent years but until now little has been known about the 
positive effect this self-care may have.


One of the work packages within the Regional Care Portal (RECAP) project 
is aiming to investigate this effect and determine whether telehealth helps 
patients to understand their long-term condition and cope better with the 
management of their disease. Started in May 2010 with funding from 
INTERREG IVB NWE programme, the RECAP project has 13 partners from 
the North West Europe region and will end in March 2015.

With leadership from Health Enterprise East, the Community Heart Failure Team at North East London NHS Foundation Trust (NELFT) is piloting the self-management of newly diagnosed heart failure patients in the community using telehealth 
Telehealth devices are given to the patients for a period of six weeks and daily physiological and psychological measures are taken and sent as anonymised data to Leuven University (KU Leuven), in Belgium, for independent analysis.
Blood pressure, pulse, oxygen saturation and weight are measured and the results show significant differences between the self-care behaviour before and after the monitoring program. Specifically the questions showing weight and fluid 
intake per day showed a significant improvement in the patients’ compliance to the program.
NELFT has redesigned how it delivers heart failure support in the community as a result of this pilot – with the inclusion of a dedicated telehealth support worker being key to realising the full benefits of the technology."
